• To enable us to assess whether our services could be of use to the person submitting the form
• To establish which kind of therapy session (if any) would be appropriate to their needs
• To assess which practice (if any) might be best due to geographical location (including therapists and/or therapy practices not part of Avon Hypnotherapy)
• To enable us to better establish whether age is going to be a determining factor in the potential efficacy of the therapies offered
• To determine the location of the practice preference
• To determine the submitter's preference in terms of the type of session or information required
• To have an (optional) telephone number to hand, so that we could respond to any queries more quickly and effectively
• To gather any further details that might help us to handle the submitter's request more quickly and effectively
• To enable us to determine the location and age demographics of those who use the web forms

• Where the person is contacting Avon Hypnotherapy with regard to a training course, we may pass such details to a third party training provider.

Any person contacting us agrees that we may contact that person (including sending follow-up e-mails) from time to time to ensure that the person who submitted the form has received a satisfactory level of service

We have secure FTP access to this information.
